One field moves, four other things become false
An engineer moves a delivery date
Stored as the pair: was the fourteenth, now the twenty-eighth, moved by a named engineer with a one-line reason.
Four dependents go stale at once
Two tasks, a paragraph in the statement of work and a client review on the twentieth now contradict a moved fact.
The account lead gets one line
Three dependents are internal cleanup. The client was told the fourteenth in writing, so that one reaches a person today.

The before is captured at the write
The app records the change with the prior value attached. Snapshots hours apart keep the net result and lose every step.
Breakage is read off the work graph
The work graph maps how tasks, documents, meetings and accounts connect. What a change invalidates is a walk over those links.
Meaning is judged, not tabled
No severity table. The same date moving is urgent on a contractual milestone, unremarkable on internal work, judged against your policy.
Three honest limits
Not version control; the full history stays in the owning tool.
It will not invent a reason nobody wrote down.
It cannot see a change in an unconnected system.
