A success flag is a self-report, so before the Brain tells a person the thing is done it goes back and looks for the record.

The action returns success
The create call reports success and hands back an identifier, which is what most assistants would narrate.
The record is looked up again
The lookup reads as the person who signed in; the write landed on a project archived that morning, so the row is not there.
The problem reaches a person while small
The success flag is flipped before the reply is drafted, and the lead reads why now, not at Thursday’s review.
Cheap, inside your data, bounded by a clock
Cheap enough to stay on
One cheap read on a short budget, because a check that slows a turn gets switched off and protects nothing.
No AI in the check
The check is a lookup, not a judgment, so it cannot inherit the misreading it is checking.
The company boundary is inside the lookup
The Brain reads as the person who signed in, so the record cannot be paired with a company the AI named.
Three honest limits
It does not check the change was correct.
An action returning nothing to look up abstains.
It proves the record was there, nothing further.
